Field Trip Chaperone Information

CHAPERONE PARKING PROCEDURES

• Due to limited parking around our perimeter on field trip days, if you are a CLEARED field trip chaperone, please follow the steps below:
  • OPTION 1:  Notify teacher if you are able to meet at the destination
  • OPTION 2:  Carpool with other chaperones
  • OPTION 3:  Blacktop parking
    1. Arrive in the staff parking lot and pull up to double gate between 8:00-8:15am and wait for staff to unlock gate and direct you onto blacktop for parking
    2. Sign-in as visitor and leave car key with front office staff (to be used if car alarm goes off or an emergency arises)
    3. Move car from blacktop immediately upon return from field trip

NOTE: PARKING ON THE BLACKTOP FOR CHAPERONES NEED TO OCCUR PRIOR TO 8:15AM TO ENSURE THE SAFETY OF OUR STUDENTS

 

FIELD TRIP CHAPERONE & VOLUNTEER REQUIREMENTS

• Annual Volunteer Registration (Google Form) 

  • Annual registration and completion of google form needed.
• Online Mandated Reporter Training
  • ​​Once you reach the end of the training, access the free exam below then submit on the google form.  
  • ONLY need to complete every two years.
• Mandated Reporter Exam 
  • Passing score is 80% or better.  Submit screenshot of completion with score to link below.
• Submit Mandated Reporter Certificate of Completion OR Exam Score Here (Google Form)
  • No hard copies will be accepted. 
• Student Emergency Card (Google Form)

• Cleared DOJ fingerprints (packets are available in the school office). 

• TB clearance (Google Form)
  • You may be able to submit a TB Risk Assessment Certificate of Completion administered by a health professional instead of a skin or blood test result.  Click here to review form to determine if you are eligible.  
  • No hard copies will be accepted. 
• New Volunteer Registration Form (Google Form)
  • Submit ONLY after you have completed all other requirements above.
